HEALTH INSURANCE

Dr. Venessa "V" Madrigal is a licensed naturopathic physician in Oregon and Washington; and in California as of October 2024!

 

They can only have appointments with residents of these states. 

 

Insurance accepted for Oregon & Washington residents; no insurance accepted for California residents. In Oregon & WA, patients are responsible for knowing their naturopathic medicine insurance benefits. They also offer a cash pay option for residents in OR & WA who do not have insurance or have an insurance plan that Dr. Madrigal does not accept; and cash pay only for CA residents. See below for cash rates.

 

Dr. Madrigal is contracted with the health insurance companies listed below for residents in Oregon & Washington:

COMMERCIAL:

  • ​BlueCross/BlueShield: Premera, Regence, Federal Employee

  • MODA

  • PacificSource - expected ~ fall 2024

  • Providence - expected

 

For LENS Neurofeedback: some non-Medicaid insurance plans cover the billing code, some only cover for certain diagnoses, some will only cover for a certain number of visits (typically only 10), and some do not cover it. If your plan does not cover code, it's an additional $40/session.

MEDICAID: only for gender-affirming care, mold-related illness or integrative mental health / LENS neurofeedback. No primary care.:

  • WA Medicaid / Apple Health / Molina: *NO* longer accepts 

  • OR Medicaid: CareOregon

    • for LENS: no additional fee as this *is* a covered service.​

  • For those with other Medicaid plans: patient to complete Client Agreement to Pay for Health Services and pay for the non-covered services out of pocket. Form would need to be filled out before each visit. Cash rate varies by clinic.

**Dr. Madrigal does not accept Medicare or Kaiser**

MILITARY VETERANS

Dr. Madrigal is a volunteer provider for Returning Veterans Project. For post-9/11 veterans in Portland Metro who have insurance other than what Dr. Madrigal currently accepts, office visits are free! These appointments are specifically for those w/ military-related PTSD and/or TBIs. Space limited per month.

CASH RATES

Oregon & WA Residents: 
 

  • First appointment, 45 min: $250

  • Follow-up appt, 30 min: $125​

California Residents: 
 

  • First appointment, 60 min: $400

  • Follow-up appt, 45 min: $300

  • Additional pre- or post-appt time to review your records, complete documentation, etc: same as first appt rate (ie OR/WA: $250/ hr; CA: $400/hr)

  • LENS sessions: no additional fee.

​

  • ​New patient hold fee: 1/2 amount of first appt fee required when scheduling. Credit card securely saved in credit card processing service for future charges.

​

  • Refunds:

    • No refund if appointment cancelled within 24 business hours prior to appt;

    • 50% refund if cancelled between 24-48 business hours prior to appt;

    • 100% refund if cancelled > 48 business hours prior to appointment. 

OR/WA Cash Rate Packages:

  • 5-pack: $600 (save $5/session)

  • ​10-pack: $1,150 (save $10/session)

CA Cash Rate Packages:

​

  • 5-pack: $1,450 (save $10/session)

  • ​10-pack: $2,800 (save $20/session)
